Job Summary and Qualifications

We are seeking a Social Worker to join our healthcare team. Your primary function is to facilitate timely and appropriate discharges from the hospital; identify inappropriate or untimely services; identify opportunities for improvements in patient care; assist patients/families in obtaining help in relation to disability, illness, financial hardship or other problems; function as a liaison with insurance/managed care/utilization review companies and provide documentation of the above per regulatory requirements and hospital policy. You will work with a phenomenal team that cares like family and works together in a collaborative manner.

In this role you will:

Demonstrate knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives; monitors self-compliance and implements process changes to ensure compliance to such regulations and quality initiatives as it relates to the provision of Case Management Services.

  • Collaborate with RN Case Managers in the plan of care of patients and families within the acute setting and along the healthcare continuum.

  • Assist the RN Case Manager with updates from insurance providers on discharge assistance that can be provided for the patient/family.

  • Act as a liaison through effective and professional communications between the Case Managers, hospital staff, outside agencies and patient/family.

  • Assist the RN Case Manager in the plan of care to ensure timely and cost-effective use of resources.

  • Assist with Baker Act placements by updating and communicating needed information to outside facilities.

  • Assist with appropriate after care needs (i.e. HHC, SNF, hospice, etc.).

  • Assist in finding community resources for unfunded patients.

  • Demonstrate compassion and care including age and culturally appropriate behavior when dealing with patients, visitors, coworkers, and others.

Job Requirements:

  • BSW or MSW from an accredited institution required

  • 1-3 years of social service experience
