Job Summary and Qualifications
POSITION SUMMARY: The Healthcare Navigator is responsible for the facilitation of patient-centered care orientation for specific service lines, education, coordination and adherence to treatment plans, access and referral to community resources and assistance with financial needs, while improving outcomes and satisfaction with care. There are four primary functions of patient navigation. The distributed weight of each function may vary depending on the needs of their assigned service line. The four primary functions are: Patient Navigation, physician relations, community events/marketing, and research/education. The navigation process is evaluated, documented and minimally reported to the GCRMC Senior Leadership team on an annual basis. Reporting requirements to regulatory agencies may also be required. The patient navigation program utilizes program data and community needs assessment information to determine necessary program improvements.
The Healthcare Navigator serves as patient advocate, partner and resource in navigating complex health issues for patients. The Healthcare Navigator must be organized, empathetic, demonstrate strong therapeutic communication skills and have a strong working knowledge of the various disciplines involved in women’s health.
What qualifications you will need:
Bachelors Degree
Basic Cardiac Life Support, or BCLS Instructor must be obtained within 30 days of employment start date
Advance Nurse Practitioner, or Registered Nurse
