Job Summary and Qualifications

  • Promotes and monitors Sepsis/Stroke program and its safety.

  • Assists and recommends protocols and guidelines for best practice for the treatment of septic and cardiovascular accident patients across the care continuum. Assists in the development of strategic plan for the growth of the organization.

  • Works collaboratively with the medical staff and hospital departments to assure that there are effective Sepsis/Stroke services in place based on evidence-based practices and national guidelines.

  • Works with hospital neurologists/cardiologists, emergency room physicians, intensivists cardiologists, and hospitalists to provide well-coordinated services to the patient.

  • Coordinates and provides direction to the core team including coordination and communications with the medical staff, nursing, and other clinical support areas by providing accurate and timely information and written communication.

  • In collaboration with the medical staff and interdisciplinary teams, develops and monitors Sepsis/Stroke practices, guidelines, order sets and clinical guidelines.

  • Serves as a resource for hospital staff on all Sepsis/Stroke related issues.

  • Co-chairs Sepsis/Stroke Committee, prepares agendas, documents disseminate minutes, follows up on and completes open items, reports to organizational quality committees, maintains sepsis and stroke data in conjunction with the Quality and Regulatory Department, provides accurate and timely clinical data to appropriate committees and identifies opportunities for improvement.

  • Responsible for initial and ongoing education programs and development of content for Sepsis/Stroke certification compliance with mandatory standards.

  • Coordinates and facilitates Sepsis/Stroke educational activities as required for physicians, staff, patients and the community.

  • Works as a liaison to EMS to assure optimal continuity of care, from pre-hospital to hospital.

  • Educates nurses and ancillary staff on best practices in the treatment of patients. Develops education for all new hires to reach mission and vision goals of organization and maintains department certification.

  • Oversees the tracking process for data related to Sepsis/Stroke patients and provides data to Sepsis/Stroke Committee to initiate changes that will drive process improvement.

  • Works closely with Quality Management to assure metrics are in place and shows continuous improvement in the care of sepsis/stroke patients and for CMS and Joint Commission reporting purposes. Reviews charts as necessary. Collects data and oversees process for data collection.

  • Aggregates and analyzes data, provides benchmarks, verifies statistical measures, understands databases and data collection methodologies specific to chest pain, oversees the development of performance improvement and action plans

  • Monitors compliance with Joint Commission certification guidelines.

  • Prepares sepsis and/or stroke incidence reports and analyzes statistical data of the Sepsis/Stroke program activities for audit review, administrative review, and physician review.

  • Abstracts and analyzing medical records. Analyzes medical records of re-admitted patients for current follow-up and/or secondary treatment.

  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.

What qualifications you will need:

Education :

  • Bachelor of Science in Nursing

Experience :

  • Minimum of five years of experience as a Registered Nurse in a care setting applicable to critical care and one year demonstrated clinical leadership role.

Licensure, Certification, and/or Registration:

  • Current FL RN license required or appropriate compact licensure.

  • BLS required

O ther Minimum Qualifications:

  • Comfortable with working with physicians, healthcare providers and other stakeholders in the organization. Capable of gaining immediate credibility with individuals through experience, presentation, communication skills, empathy, and compassion. Analytical skills including a working knowledge of basic statistics and statistical analysis methodologies. Knowledge of PC-based computer software, i.e. Word, Excel, and/or similar systems preferred. Ability to work independently and interdependently. Broad knowledge and familiarity with accreditation and healthcare-related regulatory standards
