Job Summary and Qualifications

The Oncology Clinical Pharmacist develops and leads pharmacy clinical programs for Oncology Services. Responsible for providing comprehensive, patient-centered oncology and supportive care pharmacotherapy management while serving as the chemotherapy subject matter expert for the multidisciplinary care team. This includes, but is not limited to, clinical assessment of patient antineoplastic regimens and supportive care, providing recommendations to providers based on evidence-based medicine and/or national guidelines, monitoring patient medication regimens, serving as a medication information resource for patients, the pharmacy department and all members of the oncology care team, and assisting in transitions of care and patient/caregiver education to ensure optimal care. Assists in mentoring pharmacy staff, students, and other disciplines as needed.

Manages chemotherapy processes by collaborating with the multidisciplinary team on the creation and management of order sets, policies, procedures, practice guidelines and research projects.

Actively participates in hospital committees by developing and presenting monographs, drug class reviews, and criteria for use utilizing a comprehensive review of the primary literature. Presentations pertain to clinical recommendations, pharmacoeconomic analysis, and medication safety concerns.

Participates in departmental or hospital-wide performance improvement activities as assigned. Initiates projects that will lead to continuous improvement of services provided and/or optimization of drug therapy utilization. Assist in assuring regulatory compliance, including The Joint Commission standards.

Support and participate in the goals of the organization’s quality improvement, risk management and patient safety program to include, but not limited to, National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives.

Completes notification reports regarding medication errors, adverse drug reactions and any other issue that is a risk to patient safety.

Recommends, designs, implements, monitors, and modifies pharmacotherapeutic plans including but not limited to: antineoplastic regimens, supportive care, symptom prevention and treatment, pain and palliative care, antimicrobial regimens, immunosuppression regimens, and appropriateness of prophylaxis medications and immunizations.

Actively participates in daily multidisciplinary rounds and provides team with evidence-based clinical decision support.

Educates and counsels patients and caregivers prior to chemotherapy/targeted therapy initiation to ensure comprehension of treatment plan, medication administration, and side effects

Monitors drug therapy for assigned patients and evaluates therapeutic endpoints and potential toxicities taking into account age and patient specific considerations. Intervenes when appropriate to optimize therapeutic outcomes, minimize adverse drug events, and reduce drug costs.

Actively participates in clinical pharmacy services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO conversion, renal dosing and other programs as defined by hospital leadership.

Promotes antimicrobial stewardship by monitoring and managing prophylactic and treatment regimens.

Provides drug information services for physicians, nurses, and other health care personnel including primary literature review and evaluation.

Assists with the evaluation and initiation of all clinical trial protocols for oncology patients.

Educates health care providers, nurses, and pharmacy staff routinely about current/new evidence-based practices and safe medication preparation, administration, handling, and disposal per USP 800 standards

Assists with transitions of care during each stage of a patient’s care including: medication reconciliation upon admission, discharge planning and management, facilitation of hospice/palliative care when needed, and assisting in the provision of financial aide through patient assistance programs.

Works with pharmacy team to ensure that adequate stock is maintained for treatment of the patient population and assists with drug shortage issues.

Demonstrates literacy in using computerized information systems.

Documents all clinical activities and interventions accurately and completely in Vigilanz.

Leads the training and mentorship of pharmacists, pharmacy students, residents, technicians, nurses and physicians as it relates to the oncology service line and chemotherapy/ immunotherapy education

Effectively supervises pharmacy operations activities and provides support to the entire pharmacy team

Effectively and proactively intervenes in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions

Provides on-going informal drug therapy education, in-services and/or formal education programs for medical, nursing, and allied health staff including pharmacists

Serves as a clinical preceptor for pharmacy students and residents, as assigned. Actively participates in the design of rotational experience for pharmacy students and residents.

Maintain professional competency sufficient to meet current and future needs of the facility

Has knowledge of the patient population served and applies that knowledge in the care of those patients

Train and participate with Code Teams as assigned. Obtain and maintain BLS/ACLS current certification as appropriate.

Strong patient care orientation, self- motivation, and good interpersonal, communication, and problem solving skills, and the ability to work effectively as a team player are required

Suggests appropriate, cost-effective therapeutic medication alternatives to medical staff

Assists with the hospital formulary, minimizing use of non-formulary medications, utilizing therapeutic interchanges and working with providers to promote rational and cost-effective drug therapy.

Evaluates and analyzes protocols to lower costs and optimize patient outcomes. Anticipates changes in drug therapy and report the financial impact it will represent to the overall hospital and drug budget to develop cost effective clinical pathways

What qualifications you will need:

  • Registered Pharmacist with current Texas License

  • Doctor of Pharmacy from ACPE- accredited college of Pharmacy required

  • Completion of a hospital-based PGY1 pharmacy practice residency and a PGY2 specialized residency in general oncology or hematology/oncology OR completion of a PGY1 pharmacy practice residency and Board Certified Oncology Pharmacist (BCOP) OR if no residency training, at least four years of clinical Oncology experience.

  • BLS required within 90 days of hire

  • ACLS required within 90 days of hire
