Job Summary and Qualifications

Summary:

Program Guides support the program in a wide variety of ways. Program Guides often work directly with participants (impaired adults) during the majority of their time, but may also provide program support with non-direct care tasks. All 3 levels of Program Guides are responsible for assisting participants with personal care needs (showers, incontinence, emptying catheter, dressing, etc.) and for assisting with snack and meals as needed. See attached chart for specific tasks associated with each Guide

What qualifications you will need:

  1. Program Guide III is trained in multiple areas/tasks and provides support to the program in a variety of ways.

2. Consistently provide Person Centered Care on a daily basis.

Performance Criteria:

  1. Assure that participant’s needs/preferences are being met through spiritual, physical, mental, purposeful and social activities.

  2. Assure that participant’s personal care needs are met according to his/her identified and expressed needs.

  3. Assure that participant’s nutritional needs/preferences are being met.

  4. Support team member’s understanding of their individual and group effect on participant’s experience.

  5. Support participants in being as independent as possible and allowing them appropriate self-determination.

  6. Utilize creativity in planning, creating and interacting with participants.

3. Coordinate environmental and supply needs with other team members in order to offer the optimum experience for the participants.

Performance Criteria:

  1. Set up room/space to best accommodate each activity or event; make adjustments to set up as group size changes; rearrange room after activity/event for future use.

  2. Gather needed supplies to provide optimum outcomes during activities/events; put supplies away in an organized and timely manner.

  3. Work with other team members to implement meaningful and successful special events /outings.

  4. Utilize available resources to maximize participant benefit and minimize waste to the organization.

  5. Assist in maintaining clean and orderly activity spaces/rooms, kitchen and personal care areas.

4. Performance Improvement: Continually identify opportunities to improve care for participants. Identify opportunities for organizational improvements. Systematically collect timely and accurate data to evaluate opportunities.

Performance Criteria:

  1. Assist participants and caregivers in filing grievances and appeals (PACE only).

  2. Participate in and support Quality Improvement initiatives.

  3. Participate in continuing education classes and any required staff and training meetings.

  4. Maintain participant’s medical record (PACE only) and fulfill charting/reporting requirements.
